Your rights under the California Consumer Privacy Act (CCPA)

If you are a California resident, you may opt out of the “sale” or “sharing” of your personal information.

We do not sell your personal information in the ordinary sense of the word. However, the definition of “sale” under California law is very broad and may cover common business activities that are not usually thought of as a sale — such as sharing technical information about your device with third-party advertising partners. That activity may also count as “sharing” under California law. You have the right to opt out of this type of disclosure.

Protected health information is never involved. Medical and dental information you provide to this practice is not shared with advertising partners under any circumstances, and is not covered by this opt-out because it is never disclosed for advertising in the first place. See our Privacy Policy.

How to opt out

Send a request to hello@implantwide.com with the subject line “Do Not Sell My Personal Information”. Please include the email address or phone number you have used with us, so we can identify your records. We will confirm when the request has been actioned.

By phone

Call the practice on 630-912-2700 and tell any member of staff that you wish to opt out.

Through your browser

Advertising cookies are the mechanism involved, so browser controls are effective immediately and do not require a request to us:

Block or delete cookies through your browser’s privacy settings
Reset or disable the advertising identifier on your mobile device
Send a Global Privacy Control signal, which we honour where your browser or extension sends one

Your other California rights

You also have the right to know what personal information we have collected about you, to request a copy of it, to request its deletion, and to correct inaccurate information. We will not discriminate against you for exercising any of these rights.

Requests relating to your dental record are handled under HIPAA and Illinois record-retention law, which may require us to keep certain records for a defined period even where deletion is requested.
