Privacy Policy
This Privacy Policy describes how Weingarten Group P.C. (“ImplantWIDE”, “we”, “us”, “our”) collects, uses, stores and shares your information when you access our website at implantwide.com (“Sites”), when you receive care or request information about our services, when you communicate with us in any manner, and when we work with service providers on our behalf.

By accessing or using our Sites, or by communicating with us in any manner, you confirm that you have read and understand this Privacy Policy.
Protected health information
Some of what this practice collects is protected health information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA). This includes medical and dental history, clinical photographs, radiographs and CBCT imaging, treatment records, and anything you tell us about your health through the new patient intake form, the doctor referral form, or in the course of care.
PHI is handled separately from ordinary website data:
- It is not used for marketing or advertising, and it is not shared with advertising or analytics providers.
- Notification emails sent to the practice when a form is submitted contain only a reference number, never the clinical content.
- Uploaded imaging is stored in restricted object storage, not alongside the public website.
- Access is limited to practice staff who need it in order to provide care.
Our HIPAA Notice of Privacy Practices governs the use and disclosure of PHI and is provided in the office. Where this Privacy Policy and that Notice differ with respect to PHI, the Notice of Privacy Practices controls.
What information we collect
Information you provide. Your name, email address, telephone number and mailing address; your preferred location and contact method; the content of any message, question or referral you send us; and, where you complete a clinical form, the health information described above, together with your electronic signature and consent record.
Information collected automatically. When you visit the Sites, technical information about your device is collected — IP address, browser type, operating system, referring page, pages viewed and access times. See Cookies and related technology below.
Information from third parties. We may receive information from referring dentists, from your dental insurance carrier where you ask us to verify benefits, and from service providers who help us operate the Sites.
We do not collect payment card or bank account details through this website. No form on this site takes payment.
How we use your information
- To respond to your enquiry, question or appointment request
- To provide, coordinate and follow up on dental and periodontal care
- To verify insurance benefits and administer financing arrangements where you ask us to
- To send appointment confirmations and reminders
- To send offers and practice news, where you have asked to receive them
- To operate, secure and improve the Sites
- To comply with legal, regulatory and professional obligations
Who we share it with
We do not sell your personal information. We share it only with:
- Service providers who operate parts of the Sites and our communications on our behalf, under contract and only for that purpose — our website host, our email delivery provider, and our bot-protection provider
- Other clinicians involved in your care, including your referring or general dentist
- Insurance carriers, where you have asked us to bill or verify benefits
- Legal and regulatory authorities, where required by law or professional obligation
Cookies and related technology
The Sites use cookies and similar technologies. What is set, and why, is described in full in our Cookies Policy. In summary:
- Strictly necessary — bot protection on our forms, which is required for the forms to work at all
- Analytics — Google Analytics 4, used to understand which pages people find useful
- Advertising — Google Ads, used to measure the effect of our advertising
Analytics and advertising technologies are not loaded on the new patient intake or doctor referral forms.
Most browsers let you refuse or delete cookies through their settings. Blocking strictly necessary cookies will stop our forms from working; blocking the others will not.
Your choices
- Email and text. Every marketing email carries an unsubscribe link. Reply STOP to any text message to opt out. Appointment-related messages are not marketing and continue regardless.
- Access, correction and deletion. You may ask us for a copy of the information we hold about you, ask us to correct it, or ask us to delete it. Requests concerning your dental record are handled under HIPAA and Illinois record-retention law, which may require us to keep some records for a defined period.

Security
We use administrative, technical and physical safeguards appropriate to the sensitivity of the information. Submissions are written to encrypted storage before any notification is sent, so a failure elsewhere cannot lose your enquiry. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.
Please do not email clinical detail, images or insurance identifiers. Ordinary email is not a secure channel. Use the forms on this site or call the practice.
Children
The Sites are not directed at children under 13 and we do not knowingly collect their information through the Sites. Where a minor is treated at the practice, information is provided by a parent or guardian in the course of care.
Changes
We may update this Privacy Policy. The review date at the foot of this page shows when it was last changed. Material changes will be posted here before they take effect.
