Our practice exists to take the surgical work your patients need and hand them back to you when it is done. Here is how a referral runs, and how to send one today.
Send a referral

What we need: the patient's name and contact number, the tooth or area in question, what you would like us to do, and any recent radiographs. If you have a CBCT, say so and we will arrange transfer — we would rather not repeat the exposure.

The secure route, and the one to use for imaging. Everything here travels over an encrypted connection and the files are stored privately — they are never attached to an email or put on a public address.
What you can expect
Your patient comes back to you
We treat what we were asked to treat. Restorative work returns to your chair unless you ask us to handle it — this practice is an extension of yours, not a competitor for the rest of the treatment plan.
You hear what happened
A report after the consultation and again after treatment, with the imaging. You should not have to ask your own patient what we did.
We take the difficult ones
Failing implants placed elsewhere, advanced bone loss, medically complex patients, and anyone too anxious for treatment without IV sedation.
Two locations, one record
Schaumburg and Skokie. Your patient goes to whichever is easier for them; the notes and the surgeon are the same.
